I acknowledge the following expectations for upholding academic integrity when taking an exam at the Office for Disability Services (ODS): I will not engage in cheating, plagiarism, or assist others in doing so. I will not use any unauthorized materials (e.g. cell phone, notes). Authorized materials must be documented in writing by my course coordinator, instructor, Access Manager, or Test Proctor. ODS testing spaces and computers will be monitored in-person and/or remotely via CCTV in order to ensure proper adherence to academic integrity standards. ODS will report any evidence of academic misconduct (including CCTV footage) to the Committee on Academic Misconduct (COAM) and my instructor for further review.
