Club & Organization Resources

OrganizationsThis page houses information and forms for current and prospective clubs, club members, and advisors. In order to maintain communication between club members, advisors, and the Student Activities Department, adhere to OSU policies, and to keep our Student Activities Calendar as up-to-date as possible; we encourage students and staff to familiarize themselves with the resources on this page. Completed forms can be emailed or delivered to the Student Activities Office in the Recreation Center. Please contact Heather or Sam with any questions.

Club & Organization Forms

Students or faculty seeking to officially register a club or organization should fill out the Student Organization Registration form. For existing groups, this form must be completed annually. To register for the first time, groups need a President, Treasurer, Advisor, and Constitution at minimum.  

Each academic year, registered student groups are required to host three events. One event must be social in nature and open to the ATI campus, one event should be educational to serve as a learning experience for either club members or the ATI community, and one event should be community service oriented. To assist in the planning of these events, check out the Yearly Event Club Planning form

Clubs and Organizations that plan on hosting a specific event, activity, or ordering t-shirts or other gear should fill out the Single Action Club Reporting form. If this form is being filled out for an event, make sure to check the Student Activities Calendar for any conflicting events!

For groups that would like plan ahead, the Multiple Action Reporting form is a great way to more broadly communicate your intentions for the rest of the semester.

Fundraising Information

Successful fundraisers require planning and preparation. The Fundraiser Business Plan Outline is a good place to lay out the specifics for your group’s proposed fundraiser.

Groups planning on hosting a bake sale need to be aware of applicable health regulations. The Approved Bake Sale Food Items information sheet can help. Additionally, prepared foods must contain a proper food label. View the Food Label Sample to see what this entails.

If there will be a raffle or other games at your fundraiser, view the Raffles, Drawings, and Games of Chance information sheet beforehand.
