Form Processing Instructions

Ohio State ATI is temporarily processing student requests via e-mail.  Please follow the instructions below.

NO SIGNATURES ARE REQUIRED, BUT ALL REQUESTS AND APPROVALS MUST BE SENT VIA OSU E-MAIL ACCOUNTS.

 

    Course Enrollment Permission Form – Adding a Course

    • The student must
      1. Complete the top two sections of the form and check the box indicating the reason the form is needed (i.e. prerequisite, requires permission)
      2. Do a SaveAs and name the file ADD and their last name and course number (Example: ADD Gallion 3140T)
      3. E-mail the file as an attachment to the course instructor, Jill Gallion (gallion.9@osu.edu), and Carri Gerber (gerber.140@osu.edu).  If the course to be added is internship, a copy of the approved Internship Agreement must also be attached.
    • The instructor must REPLY ALL giving their approval for the course to be added.
    • The Academic Affairs Office will then process the form and REPLY ALL to confirm that the course has been added to the student’s schedule.


    Course Enrollment Permission Form – Dropping a Course

    • The student must
      1. Complete the top two sections of the form, check the DROP THE COURSE box in the bottom section and add the LAST DATE OF ATTENDANCE
      2. Do a SaveAs and name the file DROP and their last name and course number (Example: DROP Gallion 3140T)
      3. E-mail the file as an attachment to the course instructor, their academic advisor, Jill Gallion (gallion.9@osu.edu), Carri Gerber (gerber.140@osu.edu), and if using federal financial aid, Julia Morris (morris.878@osu.edu)
    • The instructor must REPLY ALL giving their approval for the course to be dropped.
    • The academic advisor must REPLY ALL giving their approval for the course to be dropped.
    • The Academic Affairs Office will then process the form and REPLY ALL to confirm that the course has been dropped from the student’s schedule.


    Withdrawal - Dropping ALL COURSES for the semester

    • The student must
      1. Complete the form down to the COLLEGE OFFICE USE ONLY section
      2. Do a SaveAs and name the file WITHDRAWAL and their last name
      3. E-mail the file as an attachment to Jill Gallion (gallion.9@osu.edu), Carri Gerber (gerber140@osu.edu), and if using federal finanial aid, Julia Morris (morris.878@osu.edu)

    •  The Academic Affairs Office will review the form and REPLY ALL to provide notifiation of the approval or denial of the request.


    Dual Major

    • The student must
      1. Complete the form with their current major and the major they wish to add
      2. Do a SaveAs and name the file DUAL MAJOR and their last name (Example: DUAL MAJOR Gallion)
      3. E-mail the file as an attachment to their current advisor, the advisor of the major they wish to add, Jill Gallion (gallion.9@osu.edu), and Carri Gerber (gerber.140@osu.edu)
    • Both advisors must REPLY ALL giving their approval for the second major to be added.
    • The Academic Affairs Office will then process the form and REPLY ALL to confirm that the second major has been added to the student’s record.


    Petition for Alternate Finals

    • The student must
      1. Complete the form their name, OSU ID#, Major, and Course(s) for any which a time change is being requested
      2. Do a SaveAs and name the file ALTERNATE FINALS and their last name (Example: ALTERNATE FINALS Gallion)
      3. E-mail the file as an attachment to their academic advisor, the instructor(s) of the courses, Jill Gallion (gallion.9@osu.edu), and Carri Gerber (gerber.140@osu.edu)
    • The academic advisor must REPLY ALL with their approval and any comments
    • The instructor(s) must REPLY ALL giving their approval and alternate date/time/location
    • Once the advisor and instructor(s) have responded, the Assistant Director must REPLY ALL giving the final Approved / Disapproved decision.


    Petition to Change Major

    • The student must
      1. Complete the STUDENT section of the form
      2. Do a SaveAs and name the file MAJOR CHANGE and their last name (Example: MAJOR CHANGE Gallion)
      3. E-mail the file as an attachment to their current academic advisor and the advisor of their new major, Jill Gallion (gallion.9@osu.edu), and Carri Gerber (gerber.140@osu.edu).
    • Both advisors must REPLY ALL giving their approval.
    • The Academic Affairs Office will then process the form and REPLY ALL to confirm that the major and advisor have been updated in SIS.


    Petition to Substitute a Required Course

    • The student must
      1. Complete the top section of the form through the academic reason section
      2. Do a SaveAs and name the file SUBSTITUTION and their last name (Example: SUBSTITUTION Gallion)
      3. E-mail the file as an attachment to their academic advisor, program coordinator, division chairperson, Jill Gallion (gallion.9@osu.edu), and Carri Gerber (gerber.140@osu.edu)
    • The academic advisor, program coordinator, and division chairperson must REPLY ALL giving their approval.
    • Once the advisor, coordinator, and chairperson have responded, the Assistant Director must REPLY ALL giving the final Approved / Disapproved decision.
    • The Academic Affairs Office will then process the form and REPLY ALL to confirm that the substitution has been posted to the degree audit.
       

    Request to Modify Internship Requirements COVIDSP21 – waiver if graduating by Spring 2021

    Students in Associate of Applied Science programs and students in Associate of Science programs that have an internship requirement that are graduating through spring semester 2021 have the option to waive the internship requirement as long as they have the minimum 60 credit hours required by the Ohio Department of Higher Education (ODHE).

    Ohio State ATI and the College of Food, Agricultural, and Environmental Sciences (CFAES) are committed to ensuring that student progress to degree completion is not negatively impacted by disruptions related to the coronavirus (COVID-19) pandemic. At this time, internships are still being allowed. 

    • The student must
      1. Complete the top of the form
      2. Do a SaveAs and name the file MODIFY and their last name (Example: MODIFY Gallion)
      3. E-mail the file as an attachment to their academic advisor, the department chair, Jill Gallion (gallion.9@osu.edu), and Carri Gerber (gerber.140@osu.edu)
    • Both advisors and the department chair must REPLY ALL giving their approval.
    • The Academic Affairs Office will then process the form and REPLY ALL to confirm that the modification has been posted to the degree audit.


    Request to Modify Internship Requirements – using one internship in more than one program

    • The student must
      1. Complete the form through the Academic Reason section
      2. Do a SaveAs and name the file MODIFY and their last name (Example: MODIFY Gallion)
      3. E-mail the file as an attachment to the academic advisors of both programs, the department chair, Jill Gallion (gallion.9@osu.edu), and Carri Gerber (gerber.140@osu.edu)
    • Both advisors and the department chair must REPLY ALL giving their approval.
    • The Academic Affairs Office will then process the form and REPLY ALL to confirm that the modification has been posted to the degree audit.
